Document the problem without patient data.
Use the product, exact software version, non-patient error text, failed step and what you already checked. Keep patient names, scans, screenshots with identifiers and case uploads out of general troubleshooting questions.
What a useful troubleshooting note contains
- Application/module and exact installed release.
- Observed symptom or error, with identifying content removed.
- Step where it occurs and the intended non-patient task.
- Checks performed and whether authorized training data reproduces it.
- Current vendor documentation and the unresolved question.
